Who are qualified?
- Legitimate spouse of Regular Member
- Legitimate children of Regular Member, must be at least 21 years old and gainfully employed
- Philippine Veterans Affairs Office (PVAO) Pensioners, Cadets, Probationary Officers of Philippine Military Academy (PMA), Philippine National Police Academy (PNPA), Naval Officers Candidate Course (NOCC), Officer Candidate School (OCS) & Philippine Air Force (PAF) Flying School
Requirements
Spouse of Regular Members
- Marriage Certificate (PSA-certified)
- Advisory on Marriages (PSA-certified)
- Dependent’s ID and additional two (2) valid IDs
- Two (2) 2x2 ID photos
- ₱1,000 initial deposit for Capital Contribution Account
- ₱100 initial deposit for Savings Deposit Account
Children of Regular Member (within the context of marriage), 21 years old & above and gainfully employed
- Birth Certificate (PSA-Certified)
- Marriage Certificate with registry number (PSA-Certified) for married female child
- Latest Certification of Employment and Compensation using the Employer’s Letterhead (for employed individuals) or duplicate copy of latest Income Tax Return and/or Business Permit (for self-employed or with business)
- Dependent’s ID and additional two (2) valid IDs including Company ID
- Two (2) 2x2 ID photos
- ₱1,000 initial deposit for Capital Contribution Account
- ₱100 initial deposit for Savings Deposit Account
PVAO Pensioner
- Certification of Pension from PVAO or copy of latest Pension check (provided that the original copy is presented)
- Pensioner’s ID and additional one (1) valid ID
- Two (2) 2x2 ID photos
- ₱1,000 initial deposit for Capital Contribution Account
- ₱100 initial deposit for Savings Deposit Account
Candidate Officers (OCS and NOCC)
- Training Order or payslip
- Trainee’s ID or any valid ID
- Two (2) 2x2 ID picture
- ₱1,000 initial deposit for Capital Contribution Account
- ₱100 initial deposit for Savings Deposit Account
Application at the Branch Offices
- Applicant proceeds to Membership Section.
- Membership Staff screens eligibility of the Applicant based on the prevailing By-Laws and Association policies.
- Applicant fills out application forms for Membership, ATM ID Card and Mobile App, and submits documentary requirements.
- Membership Staff verifies the requirements and encodes data in the system.
- Applicant fills out Specimen Signature Card (SSC), presents valid IDs and submits 2x2 pictures.
- Membership Staff issues CCA (and SDA) passbooks.
- Applicant proceeds to the Teller Section to make an initial deposit of Php 1,000 for Capital Contribution Account and Php 100 for Savings Deposit Account. Also, member pays Php 100 for ATM ID Card fee.
- Applicant proceeds again to the Membership Section to present the Official Receipt (OR) and to process the ATM ID Card (member data capturing and printing of card).
Online Application
- Applicant visits the AFPSLAI website (www.afpslai.com.ph).
- Applicant clicks the e-Application Form (Membership Application).
- Applicant fills the required fields and waits for the reference number.
- Applicant proceeds to the Membership Section to show the reference number and submit the original requirements.
- Membership Staff verifies requirements and encodes data in the system.
- Applicant fills out application forms for Membership, ATM ID Card and Mobile App.
- Applicant fills out Specimen Signature Card (SSC), presents valid IDs and submits 2x2 pictures.
- Membership Staff issues CCA (and SDA) passbooks.

FAQs
Q: Can I submit a local registry marriage certificate?
A: No, applicant must submit a PSA-certified Marriage Certificate.
Q: I am a legal wife of a Muslim member. Do I still have the chance to become an Associate Member
since my husband’s first wife is already a member (endorsed by my spouse)?
A: While every married member can endorse his/her spouse as an Associate Member, a Regular Muslim
member can only choose one of his wives to become an Associate Member.
Q: Can illegitimate/adopted children apply for membership?
A: Yes, as long as he/she is recognized by his/her sponsor and the name of the sponsor appears on the
applicant’s birth certificate.
Q: Should the Regular Member be present when his/her dependent applies for associate membership?
A: As much as possible, the Regular Member, with whom the associate membership of the applicant is derived, is present as he/she is required to sign in the lower portion of the MAF to endorse his/her dependent, and as applicable, update his/her data with AFPSLAI.
If such is not possible, documentary proof/s that the Regular Member is still alive must be presented. Otherwise, home/hospital/etc visit may be necessary.
