REFER-CLAIMANT PROMO

ELIGIBILITY:

• All AFPSLAI members and Non-Members (herein described as “Referrer”), except AFPSLAI employees*.
• A referrer is one who is able to encourage the beneficiary of a deceased borrower (herein described as claimant) to claim CRI/Burial Assistance from the Insurance Provider through AFPSLAI.

*Project-based, CFRs, Retainers, Probationary, Regular, Board of Trustees, Corporate Officers and their relatives up to the second degree of consanguinity or affinity.

PROMO MECHANICS:

1. Interested “Referrer” may visit the nearest AFPSLAI Branch to obtain referral form/s
2. The “Referrer” must accomplish a Referral Form and present a valid ID for identification.
3. The “Referrer” and the “Referred Claimant” are both required to sign the referral form, and it must be submitted along with the necessary documents for CRI claims/Burial Assistance at the nearest AFPSLAI Branch.
4. The “Referrer” who successfully refers claimants/beneficiaries during the promo period will receive an incentive of ONE THOUSAND PESOS (Php 1,000.00). In case where the documents of the referred claimant are incomplete, the “Referrer” will not receive any incentive until the documentation is completed. All documents are subject to verification.
5. The “Referrer” will be notified by the Branch Office once the incentive is ready for release. The incentive will be released through cheque.

REQUIREMENTS:

REFERRER

1. Photocopy of two (2) valid IDs
2. Referral Form

REFERRED CLAIMANT/S

1. Original or Certified True Copy of Death Certificate
2. Original Birth Certificate or Certified True Copy of Baptismal Certificate
3. Attending Physician’s Form
4. KIA Report or Spot Report (for in line of duty)
5. Claimant’s Statement Form
6. Photocopy of two (2) valid IDs of the Claimant
7. Marriage Contract
