The bereaved family or legal beneficiaries of the deceased member applies/files for termination of membership to be able to avail ALMS benefits.

TERMINATION DUE TO DEATH

With Survivorship Agreement regardless of amount Without Survivorship Agreement Capital Contribution Account (002)/ Time Deposit Account (005) – Above Php50 Pension Account – Regardless of Amount ALMS Claim Special Requirement

Note: Additional requirements, as determined by the Branch Manager, may be required for complicated claims i.e.- multiple claimants, lack of claimants, unclear identity, etc.
  • Lost ID/Passbooks must be covered by a notarized Affidavit of Loss (AOL) which may be obtained outside or from Legal Division. If obtained outside, AOL need not pass through Legal Division. In both cases, signature of member must always be verified.
  • Valid ID includes dependent/military/ police/ pensioner’s ID, company ID, Senior Citizen Card, SSS/GSIS ID, voter’s ID, driver’s license, passport. In the absence of valid IDs NBI Clearance or other valid legal documents supporting his identity are acceptable. Postal ID acceptable only if claimant is unemployed.
  • Affidavit of Undertaking may be obtained outside or from Legal Division. It states that the account is the only money left by the decedent; he/she/they is/are the only heirs; and he/she/they will indemnify the Association for whatever damage and will keep the Association free and harmless from any liability for any claim/s that may be made later on by the legal heirs.
  • Valid ID includes dependent/military/ police/ pensioner’s ID, company ID, Senior Citizen Card. SSS/GSIS ID, voter’s ID, driver’s license, passport. In the absence of valid IDs. NBI Clearance or other valid legal documents supporting his identity are acceptable. Postal ID acceptable only if claimant is unemployed.
  • For AFP personnel, this is being issued by OTAG, For other personnel from the PNP, BJMP and civilian employees, this may be issued by their respective employer. For associate members, Certificate of Legal Beneficiaries may be obtained from previous employer in company letterhead. Alternate requirements in the absence thereof are GSIS/SSS List of Beneficiaries
